If you are travelling abroad, you
may require extra immunisations and
injections. In order that we can
provide you with these, we need to
know the full details of your travel
plans.
Please complete our
travel vaccination questionnaire at
least 8 weeks before your date of
departure. You can
download it here
or collect from reception.
The Practice Nurse
will then arrange with you a
programme to carry out any necessary
vaccinations that are required.
Please note that not all travel
vaccinations are free of charge. Any
fee payable will need to be paid at
the time of your appointment.
If your holiday is
within 2 weeks then you will need to
ring the NHS Travel Clinic on 0117
342 3400.
If you
are travelling within the next 2 to
4 weeks, please complete the form
and book a telephone appointment
with the Practice Nurse within the
next few days to discuss your
requirements.
